Employee Assistance Program

The path of life has its bumps along the way. Inevitably, you will run into problems that challenge you and disrupt your life. Most of the time you’ll be able to manage those challenges on your own. But sometimes, a problem becomes too difficult to handle alone. When you’re having trouble coping with life’s ups and downs, the Employee Assistance Program (EAP) may offer the help and support you need.

The EAP is a voluntary program that offers free and confidential assessments, short term counseling, referrals and follow-up services to employees who have personal and/or work-related problems. EAPs address a broad and complex body of issues affecting mental and emotional well-being, such as alcohol and other substance abuse, stress, grief, family problems, and psychological disorders. With just one phone call, at any hour of the day or night, you can speak with helpful resources.

Paid Time Off

Everyone needs some time off from work to fulfill family obligations, take care of personal business, enjoy a special vacation or just unwind from the demands of everyday life. You earn paid time off as soon as you begin working. It’s our way of saying that we appreciate your commitment to your job and that we’re equally committed to you and your family’s well-being.

PTO Exceptions

Employees who miss more than three consecutive unscheduled days may be required to present a doctor’s release to the Human Resources department that permits a return to work.

PTO taken in excess of the PTO accrued can result in progressive disciplinary action up to and including employment termination.

Beneficiary Assist

The loss of a loved one can leave you feeling overwhelmed. Times are difficult when the unthinkable happens. In addition to grief, you may have financial and legal worries that you are not prepared to face alone. Our Beneficiary Assist program can help you cope with the emotional, financial and legal issues that can arise after a loss. You have access 24 hours a day, seven days a week to loss counseling, financial, and legal professionals on a confidential basis.

Business Travel Accident

Business Travel Accident insurance covers you in the event of sickness, accidental death or dismemberment when traveling for business. In addition, this coverage includes:

  • 24-hour worldwide business travel protection
  • Travel assistance services
  • Emergency medical evacuation

Travel Assistance Program

With the Travel Assistance program, toll-free emergency assistance is available to you, your spouse and your dependents 24 hours a day, seven days a week when traveling 100 miles or more from your primary home for 90 days or less.

Legal Assistance

Get your legal affairs in order the easy, more affordable way. You can receive legal services for a wide variety of legal matters with access to more than 9,000 attorneys nationwide. Telephone and in-person legal consultations are available. Your coverage is portable, so you can continue to take advantage of low rates even if you leave the Company.

Home/Auto Insurance

You can take advantage of discounted home and auto insurance through Sample Provider. You may start or stop your coverage at any time throughout the year and your coverage stays with you even if you leave the Company, so you can continue to take advantage of low rates no matter where you go.

Homeowners insurance includes coverage for your house, condo or rental property (plan not available for Florida or Massachusetts residents). Auto insurance includes coverage for your automobile (including classic and antique cars), boat, motor home or recreational vehicle.

Commuter Benefits

Employees can use pre-tax dollars to pay for parking or public transportation expenses while commuting to work. Participation in the Commuter Benefits Program is voluntary and on a month-to-month basis.

To enroll in Commuter Benefits, visit SampleProvider.com to register a new account or log in to your existing account. Place or cancel your monthly order by the 10th of the current month for use in the following calendar month.

Parking Reimbursement Account

You may have tax-free money automatically deducted from your paycheck each pay period and placed in a Parking Reimbursement Account (PRA) to pay for parking expenses while you are at work. Excess balances will be carried over to the following month. However, you can only spend the amount of the monthly IRS limits ($245 for parking and $245 for transit) in any given month. For example, if you have funds roll over to the next month in your parking account and your balance is $600, you can still only spend $245 in that month (the IRS limit). You have the ability to adjust future contributions to avoid having an excess balance. Check with your Human Resources department to see how often you can change your election.

Voluntary Identity Theft Insurance

Identity theft monitoring can help prevent fraud, detect fraudulent activity on a daily basis, and resolve identity theft issues you may face. Also included is a $25,000 identity fraud insurance policy to cover out-of-pocket expenses relating to your identity theft.

Veterinary Pet Insurance

Veterinary Pet Insurance (VPI) helps offset the cost of caring for your pet with more than 6,400 medical treatments covered. VPI covers everything from preventative care to accidents and illness, as well as the costs of x-rays, office visits, medications, surgeries and hospital stays. Associates have the option of choosing their own vet or using a licensed vet in the VPI network. The cost of VPI depends on the pet’s age, species, and the coverage level selected.
